Form and Meaning Reach Ultimate Communion is a 1986 EP by the anonymous Australian band TISM. It was only ever released as an LP and is very hard to find today. When first released it was released with a picture with blue shading on the cover and a poster, the later releases had the same picture on the cover except for the red shading on the picture and no poster.
The album rose to Number 1 on the Australian Alternative Charts.
