The children return to school for the first time since the death of Ms. Choksondik in the episode The Simpsons Already Did It. During the break, Bebe (Wendy's best friend) has developed breasts. Though they're small (she originally thought they were mosquito bites), they cause all the boys in town to begin to think that she is unusually smart and cool, without even realizing why. However, this causes Wendy and the other girls to get incredibly jealous, especially since Stan, Wendy's boyfriend, is one of the boys affected.

Consequently, the boys begin to fight and bicker about who gets to play with Bebe, and first in spurts, then seemingly permanently, begin to act like cavemen. An example of the boys being attracted to Bebe is Stan, Kyle, Cartman and Tweek having a discussion on kicking out someone. Cartman at first kicks out Kyle and Tweek but Stan refuses and the next scene shows Bebe as part of the South Park four without Eric, resulting in Cartman walking over saying "Fuck you Kyle! Fuck you Stan! Fuck you Tweek! Bebe you're still cool.". Meanwhile, Bebe, worried that boys might never take her seriously (as happened to her mother, a stereotypical dumb blonde), consults a plastic surgeon about breast reduction surgery who rejects the proposition because he considers breast reduction immoral. However, when Wendy shows up asking for breast enhancement surgery, he happily obliges. Meanwhile, Stan's father has a talk with him about his breast-obsession.

The next time they are seen, at school, Bebe walks in wearing a box over her chest, which causes all the boys to suddenly break out of their trance; Stan shares his father's advice not to let breasts control their lives, and the boys all agree that it is the best. Just then, Wendy shows up, with her breast implants. Unlike Bebe, she is ridiculed rather than praised, which is probably why she doesn't have them in any later episodes (Bebe, too, appears normal in future episodes. This could mean that she found a way to cover them up).

  • In the scene where Stan, Kyle and Tweek go over to watch Terrance and Phillip with Bebe, Stan knocks on the door. Bebe's mom then answers the door. When Stan asks for Bebe, his voice sounds exactly like Kyle's.
  • Bebe says "hella-cool", which the boys perceive as cool. However, in the episode "Spookyfish". Cartman annoys everyone using the same term. Of course, this could be a sign that Bebe's breasts were clouding the boys' common sense.
  • When Cartman, Stan, Kyle, Tweek and Bebe appear in the bus stop, it appears that Bebe doesn't have any breasts.
  • Stan, Kyle and Tweek are surprised to discover Cartman plays with dolls in this episode, but in the previous episode Cartman's Mom is a Dirty Slut, they filmed him having a tea party with his dolls, so they should've already known this.

  • The main plot of this episode is a parody of the movie Slums of Beverly Hills, in which the protagonist grows breasts and gains a new level of popularity among the male crowd, and attempts to get breast reduction surgery.
  • In this episode, Cartman often plays a game called "Lambs" that is originally simply a scene out of The Silence of the Lambs.
  • Planet of the Apes and 2001: A Space Odyssey are parodied while the boys act like cavemen. 2001 was also parodied in the Trapper Keeper and Tooth Fairy Tats episodes.
  • Stan, while in his caveman persona, calls boobs "ahta", a word meaning "fire" in the language of the characters of the movie Quest for Fire.
  • The scene of Bebe's boobs conspiring while she's asleep is a parody of a similar scene involving hands in Quicksilver Highway.
  • One of the boys near the end of the episode says "Fire, bad!", referencing Frankenstein.

  • The scene near the end of the episode where Wendy gets her breast implant surgery (where the doctor haphazardly shoves the implant into her chest while blood spurts everywhere and the doctor curses profusely) was cut from the syndicated version.
